What To Expect
  • Complete eight (8) hours of comprehensive group training on location. This seminar can be completed in one day or split into two 4-hour sessions over two consecutive days.
  • Gain insights into sensitive methods for aiding patients in daily activities, recognizing triggers for difficult behaviors in elderly individuals, and implementing behavioral and care strategies to prevent or diminish challenging situations.
  • Receive essential instruction and a student handbook focused on dementia and its care, cultivating empathy and regard for affected clients while learning effective communication strategies.

Melissa Cheshire-Lynch, BSN, RN, CHPN, CDP®, CADDCT®

Melissa brings over a decade of nursing experience, specializing in hospice and critical care, to her role as Director of Hospice Services for Sussex County, DE at Delaware Hospice. With a bachelor’s degree in nursing and a slew of certifications including Certified Hospice and Palliative Nurse (CHPN), Certified Dementia Practitioner (CDP®), and Certified Alzheimer’s Disease and Dementia Care Trainer (CADDCT®), Melissa is a beacon of expertise and compassion in her field.

Her passion lies in improving care standards for patients with Alzheimer’s disease and dementia, both within Delaware Hospice and through her broader advocacy efforts, ensuring each individual receives the dignity and support they deserve throughout their journey.

CDP® Certification & CEUs

This ADDC seminar is required for all qualified individuals seeking certification as a Certified Dementia Practioner (CDP®) through the National Council of Certified Dementia Practitioners (NCCDP). Participants must apply directly for certification after completing this seminar. Additional credentialing fees apply. Association discount code and additional details will be provided in class. Discounted application pricing is $50/Large Group participant.$80/Small Group participant.

GENERAL TERMS AND CONDITIONS:

Delaware Hospice, Inc. (DHI) will be compensated for the above services at the contracted rate. Full payment of participant registration fees is due upon reservation and agreement. A separate bill will follow for travel expenses, if applicable, within 5 business days post-seminar. The balance of such fees is due within 15 days of the invoice date.

Cancellation Policy: If, for any reason, the contract is canceled by the client, $75.00 per participant will remain non-refundable. If the client cancels 7 days or less prior to the training date, the full balance of all fees is non-refundable. Should DHI cancel due to unforeseen emergencies or medical reasons, DHI will not be held responsible or liable for any costs incurred by the client, and a mutually agreeable new training date(s) will be arranged. Should DHI and the client not agree on a new training date (s), all fees collected by DHI from the client will be refunded in 30 days, excluding the non-refundable fee of $75.00 per participant. The undersigned promises to pay for services in accordance with the Terms DHI. If, at any time, for any reason, the undersigned does not pay DHI for services when due, the undersigned agrees to pay and authorizes DHI to bill the client account a service charge of 1.5% per month or the maximum allowed by law, whichever is higher. In the event that it becomes necessary for DHI to incur collection costs or legal fees associated with the enforcement of the unpaid debt, or any portion thereof, the undersigned agrees and promises to pay any and all such additional costs, charges, and expenses, including reasonable attorney’s fees if the account is placed with an attorney for collection.

Items that must be supplied by the client:

  • Projector Screen
  • LCD Projector
  • Lunch for the DHI presenter for each day of the seminar

The client agrees to comply with the following:

  1. To provide a copy of all marketing brochures or invitations for the above-mentioned program to the consultant for approval prior to distribution; once approved by DHI, no changes can be made to the marketing material.
  2. To pay DHI in full for the contracted agreement. DHI will not accept any payment from any sources other than those named as the client/representative/source on this agreement.
  3. To provide the exact number of participants and names 14 business days prior to the date of this program.
  4. If the facility pursues Certified Dementia Practitioner CDP credentials for the participants who qualify, CDP applications must be sent bundled together and sent to NCCDP directly within 30 days of completion of the seminar for a separate fee of $25.00 per application plus a 15.00 FEDEX ground fee and sent directly to the NCCDP. The conference or corporate discount letter must be attached that accompanies the CDP applications. A corporate letter must indicate all names in the packet and the address to mail the certifications with a contact name.
  5. Associations conferences are never allowed to collect CDP applications nor CDP fees and attendees will submit their CDP applications to the NCCDP (for those who qualify) at 25.00 pp and must be submitted within 30 days of the class.
  6. At no time may you advertise or call this seminar anything other than “Alzheimer’s Disease and Dementia Care Seminar.”
  7. DHI will provide copies of the class evaluations upon request.
  8. DHI will provide copies of the sign-in sheet upon request.
